Medical Emergency Agreement (*required):
In the event that any serious injury shall occur involving the student,
I wish for Jun Lu Performing Arts supervisory personnel to take appropriate steps to notify me,
but if I am inaccessible for any reason, I authorize whatever medical attention is deemed appropriate
for the student.
Other Agreements (*required):
I hereby give my consent for me and/or my child to participate in any event affiliated
with Jun Lu Performing Arts. I will remind my child of the importance of listening and
cooperating with Jun Lu Performing Arts staff and any supervision individual selected by
the Jun Lu Performing Arts personnel. 2) Performances and classes may at any time be photographed or video-taped by Jun Lu Performing Arts and I hereby consent to
the use of these materials to the discretion and ownership of Jun Lu Performing Arts.
Liability Waiver (*required):
I agree to provide health and accident insurance for the student and agree to indemnify Jun Lu Performing Arts Academy,
its Directors, Officers, employees, contractors and agents against any claim, loss, liability, damage,
cost or expense (including reasonable attorneys fees) arising out of relating to any student accident
or health issue.
I, on behalf of myself and the student, hereby agree to release, indemnify and hold harmless
Jun Lu Performing Arts, its Directors, Officers, employees, contractors and agents from any and
all claim, loss, liability, damage, cost or expense (including reasonable attorneys fees)
arising out of or relating to the enrollment, attendance, participation, withdrawal
from the programs, including but not limited to participation in classes, performances,
competitions and other activities and events offered by Jun Lu Performing Arts Academy.
